What is Support Coordination?

Support coordination is an NDIS funded service that helps you understand and use your NDIS plan. A support coordinator works with you to make sense of your funding, find and connect with providers, negotiate services and prices, and build your confidence to direct your own supports over time.

If your NDIS plan includes support coordination, it will appear under Capacity Building as Coordination of Supports (Level 2) or Specialist Support Coordination (Level 3) for participants with more complex situations.

Why participants choose Bloom Care

We are a small, Brisbane-based team with more than ten years of experience in the NDIS and disability sector. Because we keep caseloads small, your coordinator actually knows you, returns your calls, and does the follow-up work between conversations. We are independent, so our only measure of success is whether your supports are working for you.

How a support coordinator helps

  • Explains your NDIS plan and what each budget can be used for
  • Finds, vets and connects you with providers who fit your goals
  • Negotiates service agreements and prices on your behalf
  • Coordinates informal, mainstream, community and funded supports
  • Prepares you for plan reviews and helps you gather evidence
  • Steps in quickly when something is not working

Frequently asked questions

What is the difference between Level 2 and Level 3 support coordination?
Level 2, Coordination of Supports, helps you understand your plan, connect with providers and build your ability to coordinate supports yourself. Level 3, Specialist Support Coordination, is for participants with more complex situations, often involving multiple services, health systems or crisis risk, and is delivered by a more experienced coordinator with a smaller caseload.
Is support coordination funded in my NDIS plan?
Support coordination is a stated support. If it is in your plan, it appears under Capacity Building as Coordination of Supports or Specialist Support Coordination. It does not come out of your Core budget. If you believe you need it and it is not in your plan, we can help you request it at your next review.
Can I choose my own support coordinator?
Yes. You have full choice and control over who provides your support coordination, and you can change providers if your current arrangement is not working. Switching is simpler than most people expect, and we can guide you through it.
Do you also work with psychosocial disability?
Yes. Our coordinators have experience supporting participants with psychosocial disability, and we understand the extra care needed to build trust, work with mental health services and keep supports steady through ups and downs.
What areas do you cover?
We provide support coordination across Brisbane, Ipswich, Moreton Bay, Logan, the Sunshine Coast and the Gold Coast, with face-to-face, phone and video options.
How do I get started with Bloom Care?
